Fees
Our office strives to deliver the finest care at the most reasonable cost to you. Your treatment cost will depend upon the complexity of your service. During your visit, we will discuss with you the probable number of visits, their lengths, and the fees involved. Payment is due at time service is rendered. We will gladly accept the following forms of payment:

We are unable to provide in-house financing, however, we do offer an external financing option available for those who qualify. To obtain further information on CareCredit, please visit the link at www.Carecredit.com.
Insurance
We participate with the following dental insurance companies:
Aetna PPO, MetLife (PDP), United Concordia (Tricare and fee for service), and Delta Dental Premier and Non-Premier.
As a courtesy to our patients, we will gather all the necessary information regarding plan benefits and submit your claim to your dental insurance carrier. We ask that you provide us with all your insurance information at the time of your appointment. You will be informed of your estimated co-payment portion. This is the portion of the fee that your insurance company may not cover.
Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will allow 60 days for the insurance company to pay their portion. In any event that your insurance company fails to pay, rejects the claim, or does not pay the total balance, you will be responsible for the remaining balance.
If your insurance company pays more than the estimated amount, a refund check from this office will be mailed to you. If your insurance company pays less than the estimated amount, you will be billed for the remaining balance. Your prompt remittance is greatly appreciated.
Be advised insurance companies routinely indicate that coverage verification does not guarantee payment. While we try our best to estimate your out-of-pocket expense, actual payment is determined by the insurance company once they process the claim.
The contract that your employer negotiated with your insurance carrier defines your dental benefits. Various dental plans cover endodontic procedures at different payment levels and, as a result, your payment portion will vary.
If you do not have any dental insurance, or we do not participate with your dental insurance, payment is due at time of service.
We will gladly answer any additional questions or concerns you may have.
Cancellation Policy
Please note, we do request 24 hours notice for cancellations or changes to your appointment or there will be a $100 cancellation fee. To best serve his patients, Dr. Patel reserves up to 2 hours for each individual patient's treatment. Cancellations often leave this time unused, and make it difficult for us to meet the needs of other patients. It is for this reason that we request 24 hours notice. We appreciate your cooperation and understanding.
If there is a possibility that you may be running within 15 minutes late, please inform us to avoid overlapping and/or delay in other patient’s time.
